*ALL BANDS MUST SUBMIT A VIDEO or DVD of the band members performing a song they wish to perform for consideration. We will NOT be auditioning Bands live. The name of the Band, the names of all members of the band, and the song information MUST be included with this application to be considered and cannot be changed once submitted. Band will only be allowed to perform ONE song, if chosen.
LARGE DANCE GRROUPS- may submit a video in lieu of a live audition, but this MUST be the actual group performing the number that will be performed during the show and in the costumes the group will be wearing. If you prefer to audition live, the same restrictions relative to song choice and costumes applies.

Please consider my/our application for participation in Wintertainment sponsored by the Pendleton Heights High School Junior Class.
I/ we understand the artistic performance considered, will be of a nature to be shared by the entire family. I/we understand that, screening will be at the discretion of the Wintertainment committee during auditions on January 21st , if needed. If selected, I/we will be at a dress rehearsal on the evening of Wednesday, January 28th and the performance on the evening of Thursday, January 29th, 2015. I/We release South Madison School Corporation and its representatives, as well, as parents and students involved with Winterainment, of any liability for injury or theft during the dress rehearsal or performance. I/We understand that if we want to watch the show, we will need to purchase a ticket, as no complimentary tickets are given to performers since this is a fundraiser.
